What Happens If You Miss an Ozempic Dose?

If you miss an Ozempic shot, the rule from the label is simple: take it as soon as you remember if it's been 5 days or less since your usual day. If more than 5 days have passed, skip it and take the next dose on your regular day. Never take two doses to catch up. One missed dose rarely causes problems, because semaglutide lasts about a week in your body.

The 48-hour rule matters if you take a late dose close to your next scheduled one. If taking the missed dose would put the two shots less than two days apart, it's usually simpler to skip it and stay on schedule.

Semaglutide has a half-life of about a week, so after a skipped dose there's still a meaningful amount in your system. Most people notice little in the first few days. This is the situation that causes the most trouble. After two or more missed weeks, your stomach has had time to readjust, and jumping straight back to your old dose, especially 1 mg or 2 mg, can bring a rough few days of nausea, vomiting or diarrhea. Many prescribers restart at a lower dose and step back up. The Ozempic label doesn't give a specific restart schedule, so this is a call for your prescriber, not a guess.

You can move your weekly injection day if your schedule changes, as long as the last dose was at least 2 days earlier. Pick the new day and keep it going from there.
